You may dial 1-800-894-4674 or 1-800-894-4MRI between the hours of 7:00 am and 5:30 pm (PST) Monday thru Friday, or you may order on-line using our convenient Order Form.

We accept most insurance payments, workman’s comp and the following credit cards:

Remember if ordering by telephone, we ask that you have all the information ready that is asked for on the Order Form. For your convenience you may fill the Order Form out on your computer and then print it. After you print the completed Order Form you will have all the information that is required readily available for over the telephone ordering or faxing.

Faxes can be sent 24 hours a day seven days a week to our fax number at 714-736-9494.
